Booking Appointments


Monday-Friday 10 am to 6 pm; Saturday 10 am -5 pm​​

  • Taxes not included in above pricing will be included upon payment.​
  • Sales tax is currently 7.875% and will be added at the point of payment. 
  • 3% service fee for credit card payments.
  • No service fee for cash, gift certificate or credit balances on your ClinicSense.com client file.
  • The first time you schedule an appointment using ClinicSense you'll receive a confirmation email that will include a link for you to complete a brief confidential intake form online.
  • When you schedule you will also be required to enter credit card information.  This is used only to secure your appointment only.  Your credit card will not be used to pre-pay for appointments.​

You may pay with cash/check onsite or with credit card via PayPal or the ClinicSense scheduling system.​

HSA/FSA, Workman's Comp, Personal Injury insurance accepted.

Payment Options

If you need to cancel or reschedule, you will need to do so online or by calling me at least 48 hours in advance. Later cancellations or not showing for an appointment will result in a cancellation fee being charged – up to the full cost of the appointment depending on how when I receive your cancellation.
